Player Rego/Clearance Info 2009

Procedure:1 -? Print registration form off website, or can be collected in the Community Football office.Download From;

2 – Player to fill out and sign registration form and return to club registrar. If player requires a clearance from another club and league he/she must complete the clearance option provided at the bottom of the form.



3 – Club registrar to add member onto Sportsware Membership system

. If a player requires a clearance try to add the clearance first as it will pick up players previous club and history. If a clearance for a player can’t be found on system, a manual clearance is required and the completed form should be forwarded to the Community Football office who will follow through on you behalf.



4 – Original signed registration form by player and club official to be submitted to the Community Football office. Forms can either be dropped off, faxed – 6380 1322, scanned and emailed to

photos@cfwa.com.au

with a clear name and club, and or posted.



5 – Player photo identification must be accompanied with the player registration. Player photo’s MUST BE OF PASSPORT QUALITY (head & shoulder shot with white or light background) Any photo of bad quality will be refused.

6 – Player photo’s will be accepted by email;

– Camera memory cardEach individual photo MUST be fully named for easy identification by Community Football staff.



7 – No registration will be activated in Sporting Pulse until the Community Football staff have produced a Player ID card.



8 – Player ID cards can be collected in the Community Football Office.



9 – Please remember once games commence player registrations/photos must be submitted to the Community Football office by WEDNESDAY for them to be available for the weekends fixtures. No exceptions.
