Notice of Investigation (NOI)

Notice of Investigations can be lodged for a player, official, or club who is alleged to have committed or engaged in conduct that constitutes a reportable offense where a player has been injured or vilified. No payment is required with this submission. Any submissions must be received by Wednesday 9am following the match with any submissions received by Monday 9am intended for review in line with tribunals for that week. You will receive a confirmation email if this form is submitted correctly.
